Selected Reserves -- Chapter 1606

1. Complete DVA 22-1995 (Request for Change of Program or Place of Training form) online.

2. Print and complete the UMD VA Enrollment Certification Request.

3. Submit the University of Maryland Request for Certification for VA Benefits form to the University of Maryland Veterans Certification office.

Active Veterans receiving Kickers, Top Up, or Tuition Assistance must fax or mail a copy of the signed authorization form to the VA Regional Processing Office in Buffalo and submit a copy along with the University of Maryland Request for Certification for VA Benefits form to the Veterans Certification Office at the University of Maryland.
