   Enrollment Verification Request

NOTE: There is NO fee or charge associated with a request for Enrollment Verification!

 Instructions:
To request an Enrollment Verification:
  1. After you press "Submit" below, and upon entering your Student ID (SID) and PIN (Personal Identification Number), the next page will ask you to fill out:
    1. The reason for the Enrollment Verification request (i.e. employment, etc.).
    2. The semester you want verified.
    3. The type of Verification.
    4. The number of copies you would like.
    5. The address where you would like the Verification sent.
